The most common reasons an OKR shows as At Risk or Off Track:
1. No check-in in the last 14 days
OKRs Tool treats any KR not updated in 14 days as stalled. Fix: submit a check-in — even if the number hasn't changed, a check-in with a note resets the staleness flag.
2. Progress is behind expected pace
If 60% of the cycle has elapsed but your KR is only at 30% progress, the health score reflects that gap. Expected progress = (days elapsed / total cycle days) × 100.
3. Declining velocity
If your last 3 check-ins show decreasing progress (e.g. +400, +200, +100 per week), the declining velocity trend is factored in as a risk signal.
4. You manually set it to At Risk
If you selected "At Risk" or "Behind" during a check-in, this overrides the calculated health status. To reset it, submit a new check-in and select "On Track".
How to fix it
- Submit a check-in with the latest value and a note explaining the context
- If the target is no longer realistic, edit the KR and adjust the target (Admins and Managers only)
